29 CFR 1910.147(c)(1):The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: On or about April 6, 2023, the employer did not ensure that a hazardous energy control program was in place prior to allowing employees to engage in servicing and maintenance activities on the Niagara manufactured AA 5 ? press, serial no. 39910. Employees who performed set up on the specific press were exposed to crushing and amputation hazards. The employer had not developed energy control procedures or provided employees to protect them while they performed those activities. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.217(c)(1)(i): It shall be the responsibility of the employer to provide and insure the usage of "point of operation guards" or properly applied and adjusted point of operation devices on every operation performed on a mechanical power press. See Table O-10. On or about April 6, 2023, the employer did not ensure that the point of operation on the Niagara manufactured AA 5 ? mechanical power press (Serial #39910) was adequately guarded. Employees were exposed to crushing and amputation injuries while operating the machine due to the downward drift of the upper die. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
