29 CFR 1910.212(a)(3)(iii): Special hand tools for placing and removing material(s) was not provided to permit easy handling of material without the operator placing a hand in the danger zone:  Employees engaged in production activities at 2221 Industrial Rd, Nebraska City, Nebraska on or about April 29, 2023, were exposed to an amputation hazard in that the employer did not ensure that employees used a hand tool to remove rubber pieces from the DIA-LP-3 machine in the Diaphragm department. An employee had their fingers amputated when they reached in the machine.
