29 CFR 1910.178(a)(4):Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval. a) At the yard on 7900 North Florida Ave, Tampa, FL: On or about 05/08/2023, employees were exposed to struck-by and caught-in hazards, in that, a Toyota forklift Model 42-6FGCU25 is used to lift and move around granite and marble slabs in daily operations with a non-authorized Crane Jib forklift extension. b) At the yard on 7900 North Florida Ave, Tampa, FL: On or about 05/08/2023, employees were exposed to struck-by and caught-in hazards, in that, a Hyundai forklifts Model 25L-7A is used to lift and move around granite and marble slabs in daily operations with a non-authorized Crane Jib forklift extension. c) At the yard on 7900 North Florida Ave, Tampa, FL: On or about 05/08/2023, employees were exposed to struck-by and caught-in hazards, in that, a Doosan forklifts Model D70S-7 is used to lift and move around granite and marble slabs in daily operations with a non-authorized Crane Jib forklift extension.
