29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) At 5132 Citrus Leaf Blvd Winter Garden Fl 34787: an extension ladder being used to access the worksite, side rails were approximately 1 foot over the roof landing, exposing employees to fall hazards of approximately 10ft, on or about April 5, 2023.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) At 15132 Citrus Leaf Blvd, Winter Garden FL 34787: On or about April 5, 2023, the employer exposed employees to fall hazards ranging from approximately 10-27 feet to the lower level, in that employees were not using fall protection while engaged in roofing activities. Acosta Unbreakable Services, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(1), which was contained in OSHA inspection number 1582746, citation number 1, item number 1 and was affirmed as a final order on June 23, 2022, with respect to a workplace located a 1656 Wells Road, Orange Park, Florida 32073.
