29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) On or about May 8, 2023, employee(s) were exposed to amputation/entanglement hazards in-that the interlock to the Okuma MA-500HB CNC (936) was not in place to prevent employees from accessing the point of operation and other moving parts while the machine was in operation. (b) On or about May 8, 2023, employee(s) were exposed to amputation/entanglement hazards in-that the interlock to the Okuma MA-500HB CNC (934) was not in place to prevent employees from accessing the point of operation and other moving parts while the machine was in operation. (c) On or about May 8, 2023, employee(s) were exposed to amputation/entanglement hazards in-that the interlock to the Okuma MC-600U CNC(925) was not in place to prevent employees from accessing the point of operation and other moving parts while the machine was in operation. (d) On or about May 8, 2023, employee(s) were exposed to amputation/entanglement hazards in-that the interlock to the Makino a71 CNC (813A) was not in place to prevent employees from accessing the point of operation and other moving parts while the machine was in operation. (e) On or about May 8, 2023, employee(s) were exposed to amputation/entanglement hazards in-that the guarding on the Okuma (A 903) milling machine did not prevent employees from reaching the point of operation when in motion.
