29 CFR  1910.212(a)(1): The employer did not provide one or more methods of machine guarding to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   a) Starfire Swords, dba Creations, 74 Railroad Avenue, Spencer, NY 14883.  On or about  5/8/2023, the employer did not guard the rotating shaft on the Delta Bench Grinder,  model GR450, serial number 2009 13-XL 016354.  b) Starfire Swords, dba Creations, 74 Railroad Avenue, Spencer, NY 14883.  On or about  5/8/2023, the employer did not guard the rotating shafts on the Divine Brothers Company Deburrer and Buffer, marked 70381005.  c) Starfire Swords dba Creations, 74 Railroad Avenue, Spencer, NY 14883.  On or about  5/8/2023, the employer did not guard the upper and lower roller ingoing nip points on the northern most Stephen Bader Company Centerless Belt Driven Grinding and Polishing Machine, unlabeled and no identification markings.  d) Starfire Swords, dba Creations, 74 Railroad Avenue, Spencer, NY 14883.  On or about  5/8/2023, the employer did not guard the lower roller ingoing nip points on the southern most Stephen Bader Company Centerless Belt Driven Grinding and Polishing Machine, unlabeled and no identification markings.  e) Starfire Swords, dba Creations, 74 Railroad Avenue, Spencer, NY 14883.  On or about  5/8/2023, the employer did not guard the  ingoing nip point on the southern most Stephen Bader Company Contact Wheel Belt Driven Grinding and Polishing Machine, labeled 1070.   f) Starfire Swords, dba Creations, 74 Railroad Avenue, Spencer, NY 14883.  On or about  5/8/2023, the employer did not guard the rotating part, as well as guard against flying chips, on Birmingham Lathe, Model YCL-1236GH, Machine Number 631884.
