29 CFR 1910.67(c)(2)(v): A personal fall arrest of travel restraint system that meet the requirements in subpart I of this part was not worn and attached to the boom or basket when working from an aerial lift: On or about 05/02/2023, the employer did not ensure that employees working in the basket of an aerial lift were connected into the boom or basket by use of a fall protection lanyard. Employees were exposed to the hazard of falling approximately nine (9) feet.
