29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a) On or about April 14, 2023, an employee performed wood cutting operations approximately five (5) feet from the unprotected water side edge of the dock without the use of a guardrail system, safety net or personal fall arrest system. This condition exposed employees to fall hazards of greater than six (6) feet to the water below.  b) On or about April 14, 2023, employees were exposed to a break in the guardrail system of approximately four (4) feet that was used to access the lower scaffolding under the dock. This condition exposed employees to fall hazards of greater than six (6) feet to the scaffolding and water below.
