29 CFR  1910.303(b)(2): Installation and use. Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling.  a)	On or about May 9, 2023, the employer did not ensure that extension cords being used to power two 4 ft LED double light fixtures and other equipment in bay 3, were used in accordance with instructions included in the listing or labeling. Extension cords were utilized in series into a power strip, thereby exposing employees to electrical and fire hazards.   b)	On or about May 9, 2023, the employer did not ensure that extension cords being used to power a 4 ft LED double light fixture in bay 1, were used in accordance with instructions included in the listing or labeling. Extension cords were utilized in series into another extension cord (daisy chained), thereby exposing employees to electrical and fire hazards.

29 CFR  1910.305(g)(1)(iv): Unless specifically permitted otherwise in paragraph (g)(1)(ii) of this section, flexible cords and cables may not be used:  29 CFR  1910.305(g)(1)(iv)(A): As a substitute for the fixed wiring of a structure;  a) On or about May 9, 2023, in the back shop, an extension cord was used to provide electricity to two 4 ft LED double light fixtures above bay 3. The flexible cords were used in lieu of fixed wiring, thereby exposing employees to electrical and fire hazards.  b) On or about May 9, 2023, in the back shop, an extension cord was used to provide electricity to a 4 ft LED double light fixture above bays 1 and  2. The flexible cords were used in lieu of fixed wiring, thereby exposing employees to electrical and fire hazards.

29 CFR  1910.305(g)(2)(iii): Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws.  a) On or about May 9, 2023, the windshield washer fluid pump station did not have strain relief provided for its 120-volt flexible supply cord allowing wires to become visible, thereby exposing employees to an electrical shock hazard.

Sufficient access and working space shall be provided and maintained about all electric equipment to permit ready and safe operation and maintenance of such equipment.  a) On or about May 9, 2023, in the back shop, electrical control panels that controlled automotive lifts, lights, etc. were obstructed by a transmission jack, cylinder hand truck, tires and other equipment, thereby preventing employees access in the event of an emergency.
