29 CFR 1910.119(h)(3)(iv):The contract employer did not assure that each contract employee follows the safety rules of the facility including the safe work practices required by paragraph (f)(4) of this section. On or about May 5, 2023, employees were exposed to fire, explosion and toxic gas release hazards when the contract employer did not ensure employees followed Shell procedure S-102 (Permitting Procedure) to sign in the Unit Entry Log as they entered the Shell Olefin Plant #3 (OP3) process unit.
