29 CFR 1910.119(f)(1): The employer shall develop and implement written operating procedures that provide clear instructions for safely conducting activities involved in each covered process consistent with the process safety information and shall address at least the following elements.  29 CFR 1910.119(f)(1)(i): Steps for each operating phase:  29 CFR 1910.119(f)(1)(i)(G): Startup following a turnaround, or after an emergency shutdown.  a. On or about April 29, 2023, and at times prior, the employer did not ensure that the written operating procedure for starting up the steam and condensate system for the K-1 Solvent Recovery Unit covered process contained all of the necessary instructions to safely introduce steam into the process and avoid water hammer from occurring. Specific procedural steps to take along the pipeline both upstream and downstream to avoid water hammer and condensate build-up related issues include, but are not limited to, opening of necessary steam valves, traps and bleeds to allow condensate to escape as steam is slowly introduced into the system. Employees were thereby exposed to struck-by 390 degrees Fahrenheit steam, struck-by debris of broken piping, valves, and other components, as well as fire and explosion hazards while performing this task.
