29 CFR  1910.138(a): The employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasion; punctures; chemical burns; thermal burns; and harmful temperature extremes:  On or about March 30, 2026, at 42 West Buckeye Street, West Salem, OH 44287, the employer failed to ensure employees utilizing gloves had the correct sizes. An employees glove got pulled into a moving power transmission V-Belt due to being to large for their hands.

29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  On or about March 30, 2026, at 42 West Buckeye Street, West Salem, OH 44287, the employer failed to ensure employees changing O-Ring belts on the trimmer in the Pack and Inspect Department utilized lockout/tagout procedures in that they only pull the plug from the wall and do not verify power is removed.

29 CFR  1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  On or about March 30, 2026, at 42 West Buckeye Street, West Salem, OH 44287, employees were exposed to machine hazards associated with moving parts and crushing hazards while cleaning the trimmers in the Packing and Shipping Department. The employer employer failed to ensure the implement energy control application steps were followed and as a result, the applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards. As a result, employees were exposed to the unexpected energization of moving parts inside the machines.

Johnson Bros. -Greenwich, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(7)(i), which was contained in OSHA inspection number 1523887, citation number 01, item number 001B and was affirmed as a final order on September 28, 2021, with respect to a workplace located at 41 N. Center St., Greenwich, OH 44837.
