29 CFR  1910.178(f)(2):The storage and handling of liquefied petroleum gas fuel was not in accordance with NFPA Storage and Handling of Liquefied Petroleum Gases (NFPA No. 58-1969), which is incorporated by reference as specified in Sec. 1910.6.  a) In the liquid petroleum gas storage area, on or about 5/10/2023 and times prior, workers were exposed to fire hazards while performing propane change activities or walking around or near the stored tanks in that they were improperly stored on the walking working surface and unprotected against being potentially struck by powered industrial trucks.

29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) Leland Gifford drill press, S/N 10887, on or about 5/10/2023 and times prior, operators performing drill activities are exposed to amputation and caught in hazards in that the actuating foot pedal was not guarded against accidental actuation.  b) Leland Gifford drill press, S/N 10887, on or about 5/10/2023 and times prior, operators performing drill activities are exposed to amputation and caught in hazards in that the Procunier rotating chuck S/N "E" was not guarded.  c) Verson, 150 Ton press, S/N 27880, on or about 5/10/2023 and times prior, operators performing press support activities are exposed to amputation and caught in hazards in that the rear of the press was not guarded.  d) Verson, 150 Ton press, S/N 27880, on or about 5/10/2023 and times prior, operators performing press support activities are exposed to amputation and caught in hazards in that the side guard was removed to allow for continuous feed and not replaced when not in use.  e) Miller, MPS-20 AFT, S/N HK275408, resistance spot welder,  on or about 5/10/2023 and times prior, operators performing drill activities are exposed to electrical shock and caught in hazards in that the actuating foot pedal was not guarded against accidental actuation.

29 CFR  1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 PSI:  a) Throughout the plant, on or about 5/10/2023, and times prior, workers performing equipment and personal cleaning operations with compressed air were exposed to struck-by and embolism hazards in that the employer did not ensure the air nozzles were regulated to 30 PSI. and could not be dead-ended (fluted) where the output was measured at above 100 PSI.

29 CFR  1910.303(b)(1)(i):Electrical equipment was not free from recognized hazards likely to cause death or serious physical harm, in that the employer did not ensure that the electrical equipment had listing or labeling  identifying the purpose for which it was intended.  a) In the welding shop, on or about 5/10/2023, workers performing welding activities were exposed to electrical hazards of up to 230V in that the electrical outlet boxes powering equipment such as but not limited to the Lincoln and Miller welding rigs were not permanently affixed, being used as pendants.
