29 CFR  1910.252(b)(2)(i)(A): Helmets or hand shields were not used during all arc welding or arc cutting operations:  On or about May 2, 2023 and at times prior, in the warehouse welding area located at 1950 West 8th Ave, Hialeah FL 33010,  the employer did not enforce the use of personal protective equipment while performing  welding operations resulting in an employee being exposed to an arc flash hazard to the eyes.
