29 CFR 1910.119(h)(3)(iv):The contract employer did not assure that each contract employee follows the safety rules of the facility including the safe work practices required by paragraph (f)(4) of this section. On or about May 5, 2023, employees were exposed to fire, explosion, and toxic gas release hazards when the designated craft worker (Supervisor) did not walkdown the electrical isolation of MOV-2516. Although Supervisor reviewed the master isolation list and confirmed the mechanical isolation of MOV-2516, Supervisor did not visually confirm the isolation of the breaker in electrical isolation of MOV-2516 was subsequently reversed by another employer after work on E-P3-1133 had commenced.
