(a) Intersection of Jeffers Rd. and Linden Ave. in Eau Claire, WI; Cave-in protection was not provided on the north and west sidewalls of an excavation while employees install water main pipe at depths of approx. 8-feet, 6-inches in type-C soil.  The north sidewall angle of 65 degrees was too steep at a manhole riser pipe and the west sidewall was benched (vertical wall, set back and another vertical wall), which is also not allowed for type-C soil.
