29 CFR  1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l).  On or about May 8, 2023, employer did not ensure that employees operating the forklift were trained and certified to operate the forklift. This condition exposed employees to struck-by hazards.

29 CFR  1910.305(b)(2)(i): All pull boxes, junction boxes, and fittings were not provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.  On or about May 8, 2023, employer did not ensure to provide covers to the following Items:  a) Electrical switch at the 45 degree cutting machine.  b) Electrical outlet feeding the 45 degree cutting machine.   These conditions exposed employees to electrical hazards.

29 CFR  1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:  On or about May 8, 2023, employer did not ensure to install strain relief components to cables in the following instances:   a) At the extension cord feeding the router power tool. b) In the electrical box feeding the Wilson 45 degree saw.  C. At the  Wilson 45 degree saw connection.  These conditions exposed employees to electrical hazards.

29 CFR  1910.334(a)(3)(ii): Attachment plugs and receptacles may not be connected or altered in a manner which would prevent proper continuity of the equipment grounding conductor at the point where plugs are attached to receptacles. Additionally, these devices may not be altered to allow the grounding pole of a plug to be inserted into slots intended for connection to the current-carrying conductors.  On or about May 8, 2023, employer used an extension cord that was missing the ground prone connector. This condition exposed employees to electrical hazards.
