On the roof at a commercial residence, at a temporary worksite located at 4830 University Boulevard, in Jacksonville, FL: On or about May 11, 2023, and at times prior, the employer exposed employees to fall hazards, in that fall protection was not utilized by the employees while performing roofing work.

29 CFR  1926.1053(b)(1):  Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:   a. On the roof, at a commercial residence, at a temporary worksite located at 4830 University Boulevard, in Jacksonville, FL: On or about May 11, 2023, the employer exposed employees to fall hazards, in that the ladder rails did not extend 3 feet above the upper landing surface of the roof at the eave.

29 CFR  1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:  a. On the roof at a commercial residence, at a temporary worksite located at 4830 University Boulevard, in Jacksonville, FL: On or about May 11, 2023, and at times prior, the employer exposed employees to eye injuries, in that eye protection was not utilized by employees who were operating pneumatic nail guns.  Subcontractor Solutions, LLC, was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1551104, citation number 1, item number 1, and was affirmed as final order on December 28, 2021, with respect to workplace located at 1657 N. River Bluff Road, Jacksonville, FL.
