29 CFR  1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.     Location: 109 Baldwin St. Charlestown, MA  02129  On or about May 11th, 2023,  an employee was exposed to falls from heights over twenty feet to the ground while accessing a Walk-Thru Scaffold Frame system where scaffold frames that were not equipped with specifically designed and constructed ladder rungs were used as a ladder to access the upper landing platforms.

Location: 109 Baldwin St. Charlestown, MA  02129  On or about May 11th, 2023, employees were exposed to falls from heights over twenty feet to the ground while restoring a residential brick building from Walk-Thru Scaffold Frame system where the scaffold was not erected, altered, or inspected by a competent person qualified in scaffold erection, moving, dismantling or alteration.
