29 CFR  1910.176(b): Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse:  On or about May 11, 2023, racking systems used for storage of parts and materials, created a crushing and struck-by hazard, in that the system was damaged in excess of the allowable limit of  0.5 inches of deflection within a 10 foot span,  per ANSI MH16.1: 2021 'Specification for the Design, Testing and Utilization of Industrial Steel Storage Racks.'  Specifically, but not limited to the following damaged rack areas were identified:  (a) Aisle PL212A12 identified rack damage to the horizontal beam with a deflection of 9/16" spanning 5 inches horizontally. (b) Aisle PL212A25 identified rack damage to the horizontal beam with a deflection of 11/16" spanning 10 inches horizontally.
