Employees were changing the LPG tank on the company forklift regularly without safety glasses or a face shield.

On or about 9 May 2023, two employees of Meridian Electric were not using appropriate hand protection when exposed to hazards from chemicals at 2392 Grissom Drive in Saint Louis, MO 63146.
