29 CFR 1926.451(e)(1): Safe access was not provided to scaffold platforms when the platforms were more than 2 feet above or below the point of access:  a) On May 11, 2023, employees were exposed to a fall hazard of approximately 20 feet while accessing a frame scaffolded.  b) On May 11, 2023, employee were not provided with direct access and egress to upper and lower levels of frame scaffold exposing them to fall hazards of approximately 20 feet.    No abatement certification or documentation is required for this item.

29 CFR 1926.602(d):  The employer did not certify that each operator had been trained and evaluated as required by paragraph 29 CFR 1910.178(l):  29 CFR 1910.178(l)(1)(i):  The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):  a)  On May 11,2023, the employer allowed employees to use JLG R8810 forklift without ensuring they were competent to operate the equipment safely, as demonstrated by successful completion of forklift training and evaluation.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
