29 CFR 1910.29(b)(3): The employer must ensure guardrail systems are capable of withstanding, without failure, a force of at least 200 pounds (890 N) applied in a downward or outward direction within 2 inches (5 cm) of the top edge, at any point along the top rail:  At an establishment located at 805 Ogden Street in Marinette, Wisconsin;  a) On or about May 12, 2023, to August 17, 2023, the employer did not ensure the freestanding guardrail installed to protect employees from fall hazard on the southeast side of Autopour #1 was capable of withstanding a force of at least 200 pounds. The guardrail was missing a weighted base at one end and was bent and damaged to the point that the system would fail to prevent a fall into the pit beneath the autopour.  b) On or about May 12, 2023, to August 17, 2023, the employer did not ensure the freestanding guardrail installed to protect employees from fall hazard on the southeast side of Autopour #2 was capable of withstanding a force of at least 200 pounds. The guardrail was missing a weighted base at one end and was bent and damaged to the point that the system would fail to prevent a fall into the pit beneath the autopour.  c) On or about May 12, 2023, to August 17, 2023, the employer did not ensure the freestanding guardrail installed to protect employees from fall hazard on the southeast side of Autopour #3 was capable of withstanding a force of at least 200 pounds. The guardrail was missing a weighted base at one end and was bent and damaged to the point that the system would fail to prevent a fall into the pit beneath the autopour.  d) On or about May 12, 2023, to August 17, 2023, the employer did not ensure the freestanding guardrail installed to protect employees from fall hazard on the southeast side of Autopour #4 was capable of withstanding a force of at least 200 pounds. The guardrail was missing a weighted base at one end and was bent and damaged to the point that the system would fail to prevent a fall into the pit beneath the autopour.  e) On or about May 12, 2023, to August 17, 2023, the employer did not ensure the freestanding guardrail installed to protect employees from fall hazard on the southeast side of Autopour #5 was capable of withstanding a force of at least 200 pounds. The guardrail was missing a weighted base at one end and was bent and damaged to the point that the system would fail to prevent a fall into the pit beneath the autopour.  f) On or about May 12, 2023, to August 17, 2023, the employer did not ensure the freestanding guardrail installed to protect employees from fall hazard on the southeast side of Autopour #6 was capable of withstanding a force of at least 200 pounds. The guardrail was missing a weighted base at one end and was bent and damaged to the point that the system would fail to prevent a fall into the pit beneath the autopour.

29 CFR 1910.305(g)(1)(iv): Unless specifically permitted otherwise in paragraph (g)(1)(ii) of this section, flexible cords and cables may not be used for purposes prohibited by paragraphs (A) through (F) of 29 CFR 1910.305(g)(1)(iv):  At an establishment located at 805 Ogden Street in Marinette, Wisconsin:  a) On or about May 12, 2023, an extension cord near Shakeout Line #5 was used as a substitute for the fixed wiring of a structure when it provided power from an outlet to the operator's radio system through a Pyramid Power Supply. The extension cord was wrapped around steel structural members to reach the operator's area where the radio was mounted inside of a steel box to protect it from the environment.  b) On or about May 17, 2023, in the Mill Room above line 2A, an extension cord was used as a substitute for the fixed wiring of a structure when it provided power from an outlet to a retractable flexible cable reel outlet. The cable reels are used on the Mill Room lines to power items such as, but not limited to, conveyors and Non-Destructive Test (NDT) machines.  Waupaca Foundry, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1531243, citation number 2, item number 4 and was affirmed as a final order on September 22, 2022, with respect to a workplace located at 805 Ogden Street in Marinette, Wisconsin.
