29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a. Chapin Custom Molding, at a worksite located at 635 Oberlin Elyria Road, Elyria Ohio 44035: On or about May 4, 2023, the employer did not ensure energy control applications steps to control the electrical and hydraulic and pneumatic energy sources on a Sterling Blow Molding Machine # 28 were implemented prior to an employee performing processor activities such as but not limited to removing plastic from the press due to a short shot on press 28. Employees performing these work task in an energized condition were exposed to caught in hazards.
