29 CFR  1926.502(d)(6)(i): A snaphook not of the locking type was engaged (i) directly to webbing, rope or wire rope; (ii) to each other (iii) to a Dee-ring to which another snaphook or other connector is attached;(iv) to a horizontal lifeline; or (v) to any object which is incompatibly shaped or dimensioned in relation to the snaphook such that unintentional disengagement could occur by the connected object being able to depress the snaphook keeper and release itself:  Shelton site:  On or about May 10, 2023, the employer did not ensure that an employee's fall protection lanyard was directly clipped on an anchor point rated for at least 5000 pounds.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  Instance #1  Milford: On or about May 7, 2023, an employee disassembling a carnival ride was not protected from falling approximately 16 feet, 5 inches to the ground by the use of guardrails, personal fall arrest systems, or other equivalent means.  Instance #2 Shelton: On or about May 10, 2023, employees assembling a carnival ride were not protected from falling approximately 35 feet, 5 inches to the ground by the use of guardrails, personal fall arrest systems, or other equivalent means.  The Coleman Brothers Shows, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(1), which was contained in OSHA Inspection #1314380, Citation 1, Item 1a and was affirmed as a Final Order on October 1, 2018, with respect to a workplace located at 1201 Boston Post Road, MILFORD, CT.

Instance #2 Shelton: On or about May 10, 2023, employees assembling a carnival ride approximately 35 feet, 5 inches from ground level were not trained to recognize fall hazards.  The Coleman Brothers Shows was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.503(a)(1), which was contained in OSHA Inspection #1314380, Citation 1, Item number 1b and was affirmed as a Final Order on October 1, 2018, with respect to a workplace located at 1201 Boston Post Road, Milford, CT.
