On or about May 10, 2023, at the above addressed jobsite, employees were using a pump jack scaffold that was not designed by a qualified person, thereby exposing the employees to falls of approximately thirteen (13) feet above the ground and exposure to a live electrical line which had four hundred (400) volts that was right next to the vertical support of the scaffold.

On or about May 10, 2023, at the above addressed jobsite, employees performing residential siding installation operations erected a pump jack scaffold less than twenty-five (25) feet from the electrical service drop line which had four-hundred (400) volts to the house, thereby exposing the employees to electrical hazards.
