29 CFR 1910.22(d)(1): The employer did not ensure that walking-working surfaces are inspected, regularly and as necessary, and maintained in a safe condition: a) Clean Room - On or about 5/11/2023, employees were exposed to trip and fall hazards in that, the employer failed to ensure that walking-working surfaces were inspected and maintained in a safe condition prior to employees walking and working in the Clean Room when the drain grate was not provided for a drain (12" x 12" dimeter) on the floor.

29 CFR 1910.132(d)(1):The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE): a) Clean Room - On or about 4/23/2023, employees were exposed to burn hazards in that, the employer failed to ensure that workplace hazard assessment was provided for Clean Room when employees were working with the hot water (approximately 212 Fahrenheit) and not provided with proper personal protective equipment while the hot water temperature gauge was removed.
