29 CFR 1910.141(a)(3)(i): Places of employment were not kept clean to the extent that the nature of the work allowed: (a) Hanger 14, Fort Campbell, KY: On or about 5/11/2023, and at times prior, the employer exposed employees to respiratory irritation and disease such as histoplasmosis, in that bird feces were not adequately cleaned from work surfaces to include floors, break areas, tool boxes, aircraft, and any exposed surfaces inside the hanger.

29 CFR 1910.141(a)(5): Enclosed workplace(s) were not so constructed, equipped, and maintained to prevent the entrance or harborage of rodents, insects, or other vermin. A continuing and effective extermination program was not in place. (a) Hanger 14, Fort Campbell, KY: On or about 5/11/2023, and at times prior, the employer exposed employees to respiratory irritation and disease such as histoplasmosis, in that birds were not effectively prevented from entering the hanger and nesting in the ceiling areas. Further, netting installed to deter birds from nesting actually trapped birds inside the building rather than preventing them from entering which increased the amount of feces and added bird carcasses to the hazardous waste.
