29 CFR 1926.502(d)(15): Fall protection. Fall protection systems criteria and practices. Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached or shall be designed, installed, and used as follows: 1926.502(d)(15)(i) as part of a complete personal fall arrest system which maintains a safety factor of at least two and 1926.502(d)(15)(ii) under the supervision of a qualified person a) On or about May 5, 2023, the employer failed to ensure that employees were connected to anchor points capable of supporting at least 5,000 pounds and independent from the scaffold system structure. As a result, employees were exposed to a fall hazard of 24 feet.

Employers shall provide and install all fall protection systems required by this subpart for an employee, and shall comply with all other pertinent requirements of this subpart before that employee begins the work that necessitates the fall protection. a) On or about May 05, 2023, the employer did not ensure that employees working on a scaffold system performing dismantle process were provided with a complete fall protection system before employees began work. As a result, one of the employees fell approximately 24 ft. to the ground.

When the employer has reason to believe: Where inadequacies in an affected employee's work involving scaffolds indicate that the employee has not retained the requisite proficiency. a) On or about May 5, 2023, the employer failed to provide retraining for employees on fall protection and scaffold use. As a result, employees were exposed to a fall hazard while erecting, dismantling, and/or using a scaffold elevated more than 24 feet above the ground.
