29 CFR  1926.453(b)(2)(iv): Employee(s) working in an aerial lift were not standing firmly on the floor of the basket:  a.  On or about May 9, 2023, at the above addressed jobsite, an employee performing metal roofing installation operations from a JLG 400S aerial lift, more than twenty-five (25) feet above ground, was standing on the midrails of the basket, thereby exposing the employee to fall hazards.
