29 CFR 1910.147(c)(4)(ii)(D): The procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures. At 101 Danny Scott Drive, New Haven, MO, 63068, On May 12, 2023, and at times prior thereto, employees were exposed to hazards associated with the unexpected energization of equipment. The procedures for locking out/ tagging out the B-Pillar Cutline LH did not have a step to test the effectiveness of the lockout device.

The certification did not identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection. At 101 Danny Scott Drive, New Haven, MO, 63068, On May 12, 2023, and at times prior thereto, employees were exposed to hazards associated with the unexpected energization of equipment. The employer had not certified the periodic inspections at least annually. Inspections were found to be more than a year out of date.
