29 CFR 1910.184(c)(6):Slings were not securely attached to their loads: On or about April 29, 2023, the employer did not ensure that slings used to lift and arrange a metal bridge expansion joint during blasting and coating operations were securely attached to the load. Employees utilized one synthetic web sling in a cradle method on an open end protrusion from the component being manipulated.

29 CFR 1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. On or about April 30, 2023, the employer did not report an in-patient hospitalization of an employee involved in a work related injury. An employee was involved in a struck by incident on April 29, 2023 and was hospitalized later that evening. The injury was not reported to the Cleveland Area OSHA Office until May, 9, 2023.
