29 CFR  1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.  20 Saint James Ave., Lee, MA: On or about May 15, 2023 and at times prior employees were exposed to head injury from impact from falling objects by protective helmets from objects such as but not limited cut and loose shingles being dropped from the roof to the ground below.

29 CFR  1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.    20 Saint James Ave., Lee, MA: On or about May 15, 2023 and at times prior, employees engaged in residential roofing activities with unprotected sides and edges greater than 10 feet above the next lower level (ground) were not protected by either guard rails, safety net systems or personal fall arrest systems exposing them to fall from height hazards.

The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.   20 Saint James Ave., Lee, MA: On or about May 15, 2023 and at times prior employees were exposed to fall hazards as part of their assigned duties and the employer had not trained them prior to exposure to these hazards in topics such as but not limited to recognition of fall hazards, dangers of fall hazards and procedures each employee must take to prevent or minimize risk of such exposures.
