29 CFR  1910.146(d): Under the permit space program required by paragraph (c)(4) of this section, the employer did not ensure that the requirements of (d)(1) through (d)(14) were met:   (a) Building F2, pallet rack blasting and paint line area; On or about May 11, 2023, entry was made into the Viking brand media blaster to close internal doors/baffles when switching the paint line over to metal frames.  The employer's permit space program was not implemented with respect to this space prior to and during entry.

29 CFR  1910.146(g)(1): The employer did not provide training so that all employees whose work was regulated by 29 CFR 1910.146, Permit required confined spaces, acquired the understanding, knowledge, and skills necessary for the safe performance of the duties assigned under 29 CFR 1910.146:  (a) Building F2, pallet rack blasting and paint line area; On or about May 11, 2023, confined space entry training was not provided to inform employees of the hazards within each confined space, entrant requirements, attendant requirements, atmospheric monitoring requirements, rescue procedures, and how to contact emergency services for areas prior to entry of confined spaces, such as the Viking brand media blaster.

29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   (a) Building F2, pallet rack blasting and paint line area; On or about May 10, 2023, guarding was not provided for a mechanically driven paint line conveyor and carriers to protect employees from caught-in, pinch-point, and/or crushing hazards.  An employee was hospitalized for lower leg injuries they received on May 10, 2023 when their foot and ankle became caught in-between a stationary paint line rail support and carrier/trolley as it was advancing down the paint line rail.
