29 CFR 1926.201(a): Signaling by flaggers and the use of flaggers, including warning garments worn by flaggers did not conform to Part VI (Section 6) of the Manual on Uniform Traffic Control Devices: On or about 05/12/2023, the employer did not ensure that flaggers, used to control the flow of traffic, on West South Street in Dix, IL conformed to Section 6E.07.02 Flagger Procedures Support. Two (2) Flaggers used hand movements and did not use STOP/SLOW signs or an approved Automated Flagger Assistance Device to control road users approaching a Temporary Traffic Control zone.

29 CFR 1926.350(a)(10): Oxygen cylinders in storage were not separated from fuel-gas cylinder or combustible materials (especially oil or grease), a minimum distance of 20 feet or by a noncombustible barrier at least 5 feet high having a fire-resistance rating of at least one-half hour: On or about 05/12/2023, the employer did not ensure that oxygen cylinders were kept separated from fuel-gas cylinders. In the laydown yard, three (3) oxygen gas cylinders were being stored right next to two (2) acetylene cylinders.
