29 CFR  1926.453(b)(2)(v):  A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  a) Jobsite, 1200 6th Avenue, Altoona, PA 16602:  Observed on or about April 13, 2023, employees engaged in roofing activities while working in the basket of a JLG brand articulating boom lift (model number:  600AJ) where not wearing fall protection.  The employees were exposed to a fall hazard of approximately 25 feet to the ground below.

29 CFR  1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    a) Jobsite, 1200 6th Avenue, Altoona, PA 16602:  Observed on or about April 13, 2023, an employee engaged in roofing activities on the first  story roof of a three-story commercial building with unprotected sides and edges was not protected from falling by the use of guardrail systems with toeboards, safety net systems, or personal fall arrest systems.   The employee was observed working on an area of the roof that had a slope of  approximately 4 in 12 (vertical to horizontal).  The employee was exposed to a fall hazard of approximately 13 feet to the ground below.    b) Jobsite, 1200 6th Avenue, Altoona, PA 16602:  Observed on or about April 13, 2023, an employee engaged in roofing activities on the third story roof of a three-story commercial building with unprotected sides and edges was not protected from falling by the use of guardrail systems with toeboards, safety net systems, or personal fall arrest systems.   The employee was observed working on an area of the roof that had a slope of  approximately 8 in 12 (vertical to horizontal).  The employee was exposed to a fall hazard of up to approximately 25 feet to the ground below.

29 CFR  1926.503(b)(1):  The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:   a)  Jobsite, 1200 6th Avenue, Altoona, PA 16602:  On or about April 13, 2023, the employer did not maintain a written certification record of fall protection training for each employee working on the roof of a three-story commercial building.
