The employee was exposed to a fall hazard of approximately 13 feet to the ground below.    b) Jobsite, 1200 6th Avenue, Altoona, PA 16602:  Observed on or about April 13, 2023, an employee engaged in roofing activities on the third story roof of a three-story commercial building with unprotected sides and edges was not protected from falling by the use of guardrail systems with toeboards, safety net systems, or personal fall arrest systems.   The employee was observed working on an area of the roof that had a slope of  approximately 8 in 12 (vertical to horizontal).  The employee was exposed to a fall hazard of up to approximately 25 feet to the ground below.
