29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about 05/09/2023, in the production room of an establishment located at 15843 Guild Court in Jupiter, Florida, employees were exposed to an amputation hazard when operating a Kaps-All Packing Systems' Bottle Unscrambler/Auto Erector that did not have a device to stop the machine when access doors were opened.
