29 CFR  1910.178(a)(4):Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:  a) On May 16, 2023 and times prior thereto, Cascade D-Series, Carton Clamp and Cascade Push/Pull attachments were used on the Raymond electric forklift, SN 475-18-11671 and the Hyster electric forklift, SN F108V16477W to unload pallets of wine boxes.   Note: Abatement certification and documentation are required for this item.

29 CFR 1904.41(a)(1) If your establishment had 250 or more employees at any time during the previous calendar year, and this part requires your establishment to keep records, then you must electronically submit information from the three recordkeeping forms that you keep under this part (OSHA Form 300A Summary of Work-Related Injuries and Illnesses, OSHA Form 300 Log of Work-Related Injuries and Illnesses, and OSHA Form 301 Injury and Illness Incident Report) to OSHA or OSHA's designee. You must submit the information once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the forms.  (a) On May 16, 2023; Hayden Beverage Company located at 2910 E Amity Rd, Boise, ID: the employer failed to electronically submit the 2022 OSHA Form 300A by March 2, 2023.
