29 CFR  1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access.  (a) On or about May 10, 2023, at the jobsite located at N26W23977 Watertown Road, Waukesha, WI 53188, the employer did not ensure the extension ladders that employees were using to access the roof on site was extended 3 feet above the eave of the roof, exposing employees to a fall hazard of approximately 11 feet.

29 CFR  1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. Exception: When the employer can demonstrate that it is infeasible or creates a greater hazard to use these systems, the employer shall develop and implement a fall protection plan which meets the requirements of paragraph (k) of 1926.502.  (a) On or about May 17, 2023, at the jobsite located at N26W23977 Watertown Road, Waukesha, WI 53188, employees were passing shingles down from the ridge of a one-story residential duplex without the use of fall protection, exposing employees to a fall hazard of approximately 11 feet.

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a) On or about May 17, 2023, at the jobsite located at N26W23977 Watertown Road, Waukesha, WI 53188, employees were exposed to fall hazards where the employer is not training workers to recognize or avoid fall hazards and the procedures to minimize the hazard.
