29 CFR  1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, were not protected by protective helmets.  Location - 3 Cardinal Road, Danvers, MA  On or about 5/17/2023, one worker without head protection was observed picking up construction debris below where roofers were installing new roof sheathing and underlayment, placing worker on the ground at risk of head injuries from falling objects or debris.

29 CFR  1926.501(b)(13): Each employees engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b).  Location - 3 Cardinal Road, Danvers, MA  On or about 5/17/2023, four employees working on a 3-12 pitched roof at heights greater than 14', were not protected from falls. There were fall protection harnesses, ropes, and anchor points in a container on site. There were no other fall protection measures being used.
