29 CFR 1910.178(a)(5): If the truck is equipped with front-end attachments other than factory installed attachments, the user shall request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: At a facility located at N7317 Highway 42, in Sheboygan, WI; the employer did not ensure that forklifts when equipped with attachments were identified with the attachments and their effects on the vehicle's capacity when: a) On or about February 18, 2023, a man-basket was used with forklifts to elevate employees. b) During the week of May 15, 2023, a Vestil load hook fork attachment (Model LM-HP, S# S874267) was used with forklifts to lift motors.
