On or about May 17, 2023, at the jobsite, a suitable means of access/ egress was not provided to employees working in an unprotected trench that was approximately 7 feet 10 inches deep, with near vertical walls, in class c soil. Employees entered and exited the excavation by climbing the trench excavation walls.
