29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: a) Jobsite, 128 WPA Road, Salix, PA 15952: On or about April 9, 2023, employees working on the roof of a three-story residential dwelling were not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b). The employees were exposed to a fall hazard of approximately 10 feet to the level below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards: a) Jobsite, 128 WPA Road, Salix, PA 15952: On or about April 9, 2023, the employer did not ensure that each employee working on the roof of a three-story residential dwelling were trained in the recognition of fall hazards and the procedures to be followed to minimize these hazards. Employees at this jobsite were working on the roof of the house while not protected from falling.
