29 CFR 1926.451(b)(1):Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii).  Jobsite located at 1700 North Howard Street, Philadelphia, PA.- The employer did not ensure that the supported tubular welded frame scaffold used by employees to conduct demolition operations from three working platforms was fully planked between the uprights, thereby exposing employees to slip and fall hazards between approximately 7' and 16' 8".   a) From the 1st working platform of the scaffold, approximately 7' 1" above ground level.  b) From the 2nd working platform of the scaffold, approximately 13' 8" above ground level.  c) From the 3rd working platform of the scaffold, approximately 16' 8" above ground level.  Condition was observed on or about May 16, 2023.

29 CFR  1926.451(e)(1):When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access.  a) Jobsite located at 1700 North Howard Street, Philadelphia, PA.- The employer did not ensure that employees were provided a safe means to access and egress the supported tubular welded frame scaffold. Employees climbed the scaffold end frames and crossbraces to access first, second and third work platforms, approximately 7' 1", 13' 8" and 16' 8" above the ground level.  Condition was observed on or about May 16, 2023.

Jobsite located at 1700 North Howard Street, Philadelphia, PA.- The employer did not ensure employees conducting demolition activities from a supported tubular welded frame scaffold, between approximately 13' 8" and 16' 8" above the ground level, were protected from fall by the use of a fall protection system.  a) From the 2nd working platform of the scaffold, approximately 13' 8" above ground level.  b) From the 3rd working platform of the scaffold, approximately 16' 8" above ground level.   Condition was observed on or about May 16, 2023.

29 CFR 1926.454(a):The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:  a) Jobsite located at 1700 North Howard Street, Philadelphia, PA.- The employer did not train employees working from the 1st, 2nd and 3rd levels of the supported tubular welded frame scaffold, who were exposed to fall hazards between approximately 7' and 16' 8", to recognize the hazards associated with working from scaffolds and to understand the procedures to control or minimize those hazards.  Condition was observed on or about May 16, 2023.
