29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:   The employer was failing to protect employees from fall hazards. This was most recently documented on May 16, 2023, at a residential construction jobsite located at 2212 Manitou Drive, Lincoln, Nebraska.  Employees were observed installing shingles while working at a height of approximately 11 feet 4 inches measured from the ground to eave without fall protection on a residential structure.

29 CFR 1926.1053(b)(1): The employer is exposing employees to fall hazards where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   The employer was failing to protect employees from fall hazards. This was most recently documented on May 16, 2023 at a residential construction jobsite located at 2212 Manitou Drive, Lincoln, Nebraska.  Employees were observed utilizing a ladder that was not extended over the upper landing surface.
