The employer did not provide machine guarding for the rotating spindle/chuck of the Carlton vertical milling machine. (b) Machine Shop: On May 18, 2023 and times prior to, employees operate a Lagun vertical milling machine during the fabrication of mining equipment. The employer did not provide machine guarding for the rotating spindle/chuck of the Lagun vertical milling machine. (c) Fabrication Shop: On May 18, 2023 and times prior to, employees operate a Dake drill press during the fabrication of mining equipment. The employer did not provide machine guarding for the rotating chuck of the Dake drill press. (d) Mechanic Rebuild Shop: On May 18, 2023 and times prior to, employees operate a Central Machinery drill press during the fabrication of mining equipment. (b) Mechanic Shop: On May 18, 2023 and times prior to, employees operated a Cincinnati Electrical pedestal abrasive wheel grinder that was not equipped with a tongue guard which had a maximum opening of 1 and 1/2 inches on the right side and 1 inch on the left side between the grinding wheels and the peripheral safety guards.
