29 CFR  1910.147(c)(7)(i)(A): The employer failed to provide adequate training to their authorized employee(s) in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.  a.) Thermoforming Line S109 - On or about May 13, 2023, the employer failed to provide training to each authorized employee performing service and/or maintenance to machinery or equipment to ensure the employee acquired the knowledge and skills required for the safe application, usage and removal of energy control devices. Machine operators and process technicians were exposed to burn/scald and crush injuries while performing maintenance on the S109 oven to clear a plastic jam between the oven and mold former.

1910.147(d)(6):  Prior to starting work on machines or equipment that had been locked out or tagged out, the authorized employee did not verify that isolation and deenergization of the machine or equipment had been accomplished:  a. Thermoforming Line S109 - On or about May 12, 2023, employees were exposed to machine hazards associated with moving parts, pinch-points, crush and thermal hazards during servicing to unjam plastic at the S109 oven/mold area. The employer failed to verify that isolation and deenergization of the pneumatic energy to the machine had been accomplished prior to performing the servicing work.
