29 CFR 1926.501(b)(11):Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On or about May 18, 2023, at the construction site on the westside of the apartment complex where employees were exposed to a 30 foot fall hazard when performing roofing activities on a residential structure without a fall protection system.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about May 18, 2023, the construction site on the west side of the apartment complex where employees were exposed to a 10 foot fall hazard while using extension ladders to access a roof without extending the ladders side rails at least three feet above the roof landing or securing the top of the ladders to the roof landing.
