On or about May 18, 2023, in the frit making area, employees were not protected from stepping into or tripping into the openings on the top of the closed pit doors. b. On or about May 18, 2023, in the frit making area, employees were not protected from stepping into or tripping into the opening of the bucket after it was lowered into the pit with the pit doors open. On or about May 11, 2023, an employee's foot went into the water of the bucket after it was lowered in the pit and sustained 2nd and 3rd degree burns.
