At 9297 97 Ln, Live Oak, Florida, in the Feed Loading Building: On or about May 10, 2023, the employer exposed three employees seven-feet to twenty-three-feet fall hazards from the Tender Blender Mixer cover, in that the employees were not protected by a fall protection system when working on the mixing blender.

At 9297 97 Ln, Live Oak, Florida, in the Feed Loading Building: On or about May 10, 2023, the employer exposed three employees to an amputation hazard, in that the employees clearing jams on the Tender Blender Mixer did not apply lock out to the equipment themselves to prevent energization of the mixing paddle before entering the mixer hazard area.

At 9297 97 Ln, Live Oak, Florida, in the Feed Loading Building: On or about May 10, 2023, the employer exposed three employees to amputation hazards, in that three employees were taking turns cleaning inside the hazard area of the Tender Blender Mixer without each employee having the protection of a personal lockout device to protect them from energization of the mixing paddle.

Adequate first aid supplies shall be readily available. a. At 9297 97 Ln, Live Oak, Florida, in the Feed Loading Building: On or about May 10, 2023, the employer exposed employees to amputation hazards from machinery, and the employer had not trained personnel to render First Aid at the feed mill. The feed mill was not in close proximity to a treatment facility.
